Looking for a Health Partner of Northern California Provider near you? Well, you’ve come to the right place. Here you can locate any provider in our network, including primary care physicians and specialists, laboratories, testing facilities, physical therapy providers, hospitals, and community health providers. All you have to do to find a provider is to select the provider type you are looking for. Health Partners of Northern CA provides members with a comprehensive provider network. As an address and/or participation status can change, we recommend that you verify the current address and participation status with any hospital or other medical facility or provider you plan to visit. This information is not intended to be either a recommendation or an endorsement of any individual health care provider or facility.

For medically necessary, covered services, Stanislaus County Employee Members enrolled in a health plan within the HPNC network can access medical and behavioral health providers outside of Stanislaus County that participate in the First Health Complimentary Network: First Health Directory Search
To enroll in Health Partners of Northern CA, or change health plans, eligible employees may sign up at any annual open enrollment. The only other time you may make changes to your benefits throughout the calendar year is if you have a Life Event. Refer to the Employee Benefits Policy for information regarding Life Events. If you are not making any changes to your health insurance plans for the year, you will not be required to complete an enrollment.
Health Plans
Health Partners of Northern CA offers both an EPO (Exclusive Provider Organization) and an HDHP (High Deductible Health Plans). The plan you participate in determines your specific benefits. To get the most value from your benefit plan, we encourage you to take the time to review and understand your options.
EPO
The EPO plan requires higher employee premiums, but has no deductible for most services so the plan starts paying benefits earlier than the HDHP. You are not eligible for an HSA or HSA employer contributions if you participate in the EPO.
HDHP
The HDHP (High Deductible Health Plan) features lower employer premiums and a higher annual plan deductible. The HDHP is offered along with a Health Savings Account (HSA) to which both you and your employer may make pre-tax contributions. Funds deposited into your HSA may be used to pay deductibles and other eligible expenses if you participate in the HDHP. Unused funds in your HSA roll-over from year to year and may be used for your future years’ medical expenses.

The goal of our Quality Improvement program is to work with local providers and national recommendations in order to determine the best clinical practice guidelines.
